At West Hampstead Medical Centre, we are seeking a highly skilled Social Prescribing Link Worker to join our team.
About the RoleWe offer a competitive salary of £28,000 - £38,000 per annum, depending on experience, plus benefits including NHS Pension, cycle to work scheme, and health and wellbeing support.
The successful candidate will have the opportunity to work with a diverse range of patients, developing personalized care plans that address their unique needs and circumstances. With a focus on social prescribing, you will work closely with local agencies, community groups, and statutory services to provide holistic support and promote positive change in patients' lives.
As a key member of our team, you will be responsible for:
- Collaborating with clinical teams to manage caseloads and conduct home visits when necessary;
- Discussing patients' health and wellbeing needs, setting personalized goals, and directing them to appropriate services;
- Connecting patients to community groups, charities, VCSE organizations, and statutory services to ensure a holistic approach to care;
- Maintaining and updating knowledge of local services, building relationships with providers, and staying informed of community developments;
- Establishing health and wellbeing support groups within the West Camden PCN, working alongside other Social Prescribers.
