About the Role

We are seeking an experienced and skilled Community Nurse to join our team at Sussex Community NHS Foundation Trust. As a Community Nurse, you will be responsible for delivering high-quality care to patients in their own homes, community hospitals, and other community settings.

Key Responsibilities
  • Deliver comprehensive care to patients with a range of conditions, including diabetes, wounds, and palliative care.
  • Work closely with patients, families, and carers to develop and implement personalized care plans.
  • Conduct regular assessments and reviews to ensure patients receive the best possible care.
  • Collaborate with other healthcare professionals to ensure seamless care transitions and coordination.
  • Participate in education and training to maintain and develop your skills and knowledge.
Requirements
  • Registered Nurse qualification and experience in a community setting.
  • ENB 998 or equivalent qualification.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong problem-solving and critical thinking skills.
