Job Overview
Care Act compliant assessments are required in acute Hospital settings. As a Home Based Social Worker, you will work with highly skilled professionals to prioritize casework and provide exceptional recording skills.

About the Role
This full-time position requires working Monday-Friday, 37 hours per week. You will have flexibility around home working, but regular visits to Hospitals for assessment completion and use of Hull Royal as a base (x2) are expected.

Key Responsibilities
- Conduct Care Act compliant assessments in acute Hospital settings
- Provide clear, concise decision making reflected on financial panel papers
- Manage time effectively in busy Hospital environments with continuous flow and prioritization of casework

Requirements
- Reliable transportation for travel between Hospital sites
- Exceptional recording skills with experience of multi-agency working
- Understanding of Continuing Healthcare criteria and DST checklists

Benefits
- Salary: £35,000 - £45,000 per annum depending on experience and location
