Job Overview

We are seeking a highly skilled and compassionate Care Coordinator to join our team at Pennine Care NHS Foundation Trust. As a key member of our Community Mental Health Team, you will play a vital role in providing integrated health and social care to individuals with moderate to severe mental health problems.

As a Care Coordinator, you will be responsible for assessing the needs of service users, developing and implementing care plans, and coordinating with multidisciplinary teams to ensure seamless care delivery. Your expertise in risk assessment, case management, and therapeutic interventions will be invaluable in supporting our service users to achieve their goals and improve their overall well-being.

Main Responsibilities

Requirements
  • Registered Mental Health Nurse, Social Worker, or Occupational Therapist
  • Experience working with individuals with severe and enduring mental health issues
  • Strong risk assessment and case management skills
  • Ability to work effectively in a multidisciplinary team
