Job Overview

We are seeking a highly skilled and compassionate Care Coordinator to join our team at Pennine Care NHS Foundation Trust. As a Care Coordinator, you will play a vital role in providing integrated health and social care to individuals with moderate to severe mental health problems.

Key Responsibilities:

  • Assess the needs of service users and develop personalized care plans
  • Coordinate and deliver individual packages of care to a caseload of service users
  • Work collaboratively with the multi-disciplinary team to ensure the needs of service users are met
  • Provide clinical supervision and support to junior staff members
  • Participate in the CMHT's duty system and respond to service users' immediate needs

Key Requirements:

  • Registered Mental Health Nurse, Social Worker, or Occupational Therapist with experience in mental health practice
  • Ability to work effectively as part of a multi-disciplinary team
  • Effective risk assessment skills and ability to complete funding applications
  • Therapeutic and counseling skills, with a focus on promoting the physical health and wellbeing of service users
