About the Role

We are seeking a highly skilled and experienced Community Mental Health Nurse to join our team. As a Community Mental Health Nurse, you will be responsible for providing high-quality care to individuals with mental health conditions in their own homes and within various accommodation settings.

Main Responsibilities
  • Assess and manage risks associated with clients with enduring mental health needs.
  • Deliver crisis interventions and offer support with mental health difficulties.
  • Monitor physical health and support social needs.
  • Support the Depot Clinic, Clozapine clinic, and Duty system.
  • Engage with community resources and external agencies to aid patients' recovery.
  • Administer and monitor the side effects of medication according to NMC professional standards.
Requirements
  • Registered Mental Health Nurse.
  • Evidence of continuing professional development.
  • 2 years community experience.

Person Specification
  • Minimum of 3 years post-registration experience.
  • Experience of supervising junior staff.
