Care Planning Specialist

The Field Care Manager will be responsible for delivering high-quality home care services to our clients in the North West region. This role requires strong leadership skills, excellent communication, and the ability to work independently.
Responsibilities- Care Planning & Assessment: Develop and review personalized care plans to ensure clients receive the best possible care.
- Liaise with operational teams regarding performance issues and training needs.
- Compliance & Quality Assurance: Ensure services meet CQC standards by conducting audits and maintaining accurate records.
- Build relationships with clients and their families to address concerns and update care needs.
- Collaborate with healthcare professionals to provide holistic care solutions.
- NVQ Level 3 or 4 in Health & Social Care (or equivalent).
- Experience in a supervisory/management role within health and social care.
- A passion for delivering high-quality care, exceptional communication, and organizational skills.
- Full UK driving license and ability to travel using own car.
- This role requires travel around the North West including North Wales, Liverpool, Manchester, Stoke, and surrounding areas.
- A competitive salary of £27,000 per annum, depending on skills and experience.
- Flexible working hours, including some evenings and weekends.
- Annual leave - 25 days plus bank holidays.
