About the Role

As a Reablement Support Worker, you will work with a warm and friendly team to provide person-centered care and support to individuals in their own homes. Your primary responsibility will be to assist individuals in managing daily living skills, including personal and intimate care, and to help them set and achieve clear and achievable goals to rebuild their independence.

Key Responsibilities
  • Support individuals in their own homes to manage daily living skills, including personal and intimate care
  • Help individuals set and achieve clear and achievable goals to rebuild their independence
  • Work collaboratively with the team to provide person-centered care and support
  • Manage medication and support with meal preparations as needed
