Award Winning Social Worker

We are seeking a highly skilled and dedicated Qualified Social Worker to join our Adult Assessment Team in Bromley. As a key member of our team, you will be responsible for providing assessments under the Care Act 2014 and working with young people transitioning from Children's services to Adult services.
Key Responsibilities- Conduct thorough assessments of vulnerable adults to identify their needs and develop support plans
- Work closely with multi-disciplinary teams to ensure effective care management and safeguarding
- Provide early intervention to prevent social admissions to hospital and support adults and carers to maintain independence
- Develop and maintain excellent relationships with service users, carers, and partner agencies
Salary: £30-40 per hour (dependent on experience and pay scheme selected)
Contract Type: 37 hours per week, 3 Month Temporary Contract
Requirements: A Degree or equivalent, registration with Social Work England, current DBS, and a car driver's license
