About the Role:
Hc-one is seeking a skilled Dementia Care Practitioner to join our team and deliver exceptional care services to individuals with dementia.
Responsibilities:
- Develop and implement person-centered care plans that cater to the unique needs of residents with dementia.
- Provide emotional support and companionship to residents, promoting their dignity and well-being.
- Collaborate with multidisciplinary teams to ensure comprehensive care delivery and address any emerging concerns.
- Maintain accurate records of resident interactions and progress.

About You:
To succeed in this role, you must possess:
