About the Role

As a Community Health Specialist, you will be responsible for delivering high-quality care to clients with complex needs in the community. This involves conducting initial assessments, training and assessing workers' competencies, and collaborating with the wider multidisciplinary team.

The ideal candidate will have advanced clinical skills in complex care, including ventilator and tracheostomy management, and experience delivering care to both adult and paediatric service users.

Key Responsibilities
  • Conducting initial assessments in the community
  • Training and assessing workers' competencies and allocating support workers to patients
  • Collaborating with the wider multidisciplinary team (MDT)
  • Attending meetings and making referrals
  • Managing a team of non-clinical case managers
What We Offer
  • A competitive annual salary of £44,000 per annum
  • £3,000 car allowance
  • Mileage expenses
  • Full-time hours: 37.5 hours per week, Monday to Friday

  • You are an RGN or RSCN with advanced clinical skills in complex care
  • You have experience delivering care to both adult and paediatric service users
  • You are a car driver, flexible to travel around the Birmingham/West Midlands area
