Salary: £38,890 - £44,503 per annum (dependent on experience)

Job Description:

We are seeking an experienced Community Nurse Specialist to join our team. As a Community Nurse Specialist, you will work in partnership with children, families, and carers to provide holistic care and support. You will be responsible for assessing, planning, and delivering individualised care plans, as well as providing symptom management and non-medical prescribing.

Key Responsibilities:

  • To work collaboratively with the multi-disciplinary team to assess, plan, and deliver high-quality patient care.
  • To provide timely, effective symptom management and non-medical prescribing.
  • To maintain accurate, concise, and contemporaneous records adhering to BCHC record-keeping policies.
  • To provide flexible care to accommodate changing patient needs.
  • To reassess patients regularly, ensuring formal reviews of caseloads are documented and evidenced.

Requirements:

  • Degree in Nursing or equivalent qualification.
  • Registered with the NMC.
  • Experience in community nursing, preferably in paediatrics.
  • Non-medical prescribing skills.
