Job Overview
We are looking for an experienced professional with excellent communication and collaboration skills to join our multidisciplinary team at Archer unit, a 20-bedded Physical Rehabilitation Unit. The successful candidate will be providing care for mainly elderly patients with a wide range of medical and surgical conditions.
The role involves supporting and facilitating appropriate rehabilitation opportunities, safe discharges, and promoting patient independence. Due to the nature of our patients' conditions, they all require complex discharge planning, involving regular liaison with therapists, social workers, and the palliative care team when necessary.
The post holder will be responsible for supporting the unit in the management and delivery of individualized care to patients admitted to the unit. This includes identifying subtle changes in condition and taking appropriate clinical actions, communicating effectively with the multidisciplinary team, and ensuring care is delivered in a safe and effective manner.
The ideal candidate will have acquired some skills and experience from their career so far and will be familiar with and efficient in emergency procedures pertaining to community inpatient settings. They will also be able to handover work to the multidisciplinary team in an efficient manner, prioritizing workload effectively, and using appropriate and agreed tools.
Main Responsibilities
