Personal Support Assistant

We are looking for a caring and dynamic individual to join our team as a Support Coordinator in our Preston service. This is an exciting opportunity to be part of our well-established services, which support individuals with learning disabilities.
Key Responsibilities:
- To provide person-centred support to individuals with learning disabilities
- To work closely with colleagues and external agencies to ensure the best possible outcomes for individuals
- To maintain accurate and up-to-date records of individual's support plans and progress
