About the Role

We are seeking a highly skilled and experienced mental health professional to join our team as a Care Coordinator for our Early Intervention in Psychosis Service.

The successful candidate will have a strong background in working with individuals who experience psychosis and will be able to provide high-quality care coordination to patients identified as having an At Risk Mental State or First Episode Psychosis.

The role will involve working closely with our multi-disciplinary team to assess, plan, and implement care plans that promote recovery and well-being for our patients.

Main Responsibilities

  • To have care coordinator responsibility for up to 20 patients.
  • To work as an integral part of the multi-disciplinary team; understanding and supporting the roles and contributions of colleagues in facilitating recovery within the Early Intervention philosophy and model of care.
  • To have knowledge, interest, and experience of working with people who experience psychosis and be familiar with the current standards and targets for Early Intervention Services nationally.

About Us

The Black Country EIP service is comprised of four EIP teams located in Dudley, Walsall, Sandwell, and Wolverhampton.

We are a specialist community mental health team working with young people and adults in the three years following FEP, or those who are deemed at risk of developing psychosis for a year.

We provide individualised, comprehensive, evidence-based interventions to optimise recovery, prevent relapse, and help individuals and their families to cope with their experiences.
