We are seeking a highly skilled and compassionate Healthcare Access Navigator to join our team at HMP Aylesbury, playing a vital role in promoting healthcare access and reducing distress and risk among individuals in custody.

About the Role:

You will work collaboratively with healthcare providers and prison staff to identify and address the needs of individuals in custody, providing reassurance, guidance, and signposting to essential services. Your excellent communication and interpersonal skills will empower patients and their families/carers, promoting timely access and positive patient outcomes.

Your Key Responsibilities:

  • Conduct regular check-ins with new receptions, providing a visible presence and accessible support.
  • Develop effective partnerships with HMPPS and healthcare providers, driving improvements in joint care and patient outcomes.
  • Maintain accurate records and logs, ensuring seamless communication and referral processes.

Requirements:

  • Strong knowledge of healthcare systems and protocols, with experience working in a secure environment.
  • Excellent communication and interpersonal skills, with the ability to build trust and rapport with diverse groups.
  • Ability to work under pressure, managing competing priorities and deadlines.

Salary and Benefits:

This exciting opportunity offers a competitive salary of £26,530 to £29,114 per annum, along with a comprehensive benefits package. The position is subject to a Disclosure and Barring Service (DBS) check, as per the Rehabilitation of Offenders Act (Exceptions Order) 1975.
