About the Role

We are seeking a highly skilled and experienced Consultant in Community Medicine for Older Adults to join our team at the NHS. As a key member of our department, you will play a vital role in providing high-quality care to older patients across our hospital sites and community settings.

Key Responsibilities
  • Provide specialist opinion and expertise in community medicine for older adults
  • Participate in ward rounds, MDT meetings, and outpatient clinics to ensure seamless care coordination
  • Develop and implement quality improvement initiatives to enhance patient outcomes and experience
  • Collaborate with multidisciplinary teams to provide comprehensive care to older patients
  • Contribute to teaching and training programs to develop the skills of junior doctors and other healthcare professionals
  • Engage in regular audit and data collection to inform service improvements
  • Maintain overview of all ward patients and actively participate in discharge planning
  • Support and supervise junior doctors on the ward
  • Liaise with Same Day Emergency Care ANPs as needed
  • Contribute to electronic and verbal handovers of patients
Requirements
  • Recognized medical degree and GMC registration
  • MRCP or equivalent qualification
  • Entry on the GMC specialist register via CCT/CESR/ECR
  • Proven experience in community medicine for older adults
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a multidisciplinary team
  • Strong leadership and management skills
  • Commitment to quality improvement and patient safety
