Adult Social Care Liaison Officer

As a Care Homes Liaison Officer at We Manage Jobs (WMJobs), you will play a vital role in ensuring referrals into our Brokerage service meet quality standards. This involves liaising with Adult Social Care Practitioners to ensure a seamless experience for our clients.
Key Responsibilities- Care Sourcing: Utilise council policies and systems to obtain suitable care offers within contractual frameworks.
- Care Establishment: Arrange care for various needs, including residential, nursing, home care, day care, supported living, and mental health.
- Resident Engagement: Discuss brokerage roles with residents, arrange provider assessments, and agree on care offers, including financial contributions.
- Documentation: Produce and send contractual documentation before care commencement and maintain professional records of discussions, assessments, and decisions in line with policies.
- Risk Management: Identify and manage risks, using appropriate escalation channels.
- Legislative Knowledge: Have a deep understanding of the legislative and policy context of Adult Social Care (Care Act) and the needs of Adult Social Care users.
- Proficiency in IT systems and software, with high accuracy in record-keeping.
