Key Responsibilities:
  • Coordinate daily activities, including medication administration, meal preparation, and social events.
  • Implement care plans tailored to individual resident needs, working closely with healthcare professionals to ensure effective delivery.
  • Communicate effectively with residents, families, and staff to ensure open channels of information and address concerns promptly.
  • Maintain accurate records, reports, and statistics to track resident progress and program effectiveness.

Requirements:
  • Minimum 1 year of experience in elderly care coordination or a related field.
  • Degree in Nursing, Social Work, or a related field (preferably with a specialization in gerontology).
  • Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
  • Basic computer skills and familiarity with software applications used in healthcare settings.
