We are seeking a Compassionate Care Assistant to join our team in Leeds, UK. The successful candidate will be responsible for delivering high-quality care services to individuals in our community, promoting independence, and fostering positive relationships with service users and carers.

Key Responsibilities:

  • Support people to meet their outcomes, involving families and professionals.\
  • Deliver personalized care through individual choice and control (when required).\
  • Provide personal care that protects people's dignity.\
  • Document, report, and share information with the right people at the right time.\
  • Foster positive relationships with service users, carers, and other stakeholders.\

  • Participate in regular team meetings and contribute to decision-making processes.\
  • Adhere to company policies, procedures, and protocols at all times.\
  • Work collaboratively with colleagues to achieve shared goals and objectives.\

  • Be proactive in identifying and addressing any concerns or issues related to care delivery.\
  • Develop and maintain knowledge of local resources, services, and support networks.\
  • Collaborate with healthcare professionals and other agencies to ensure seamless care coordination.\
  • Keep accurate and up-to-date records of client interactions and progress.\
  • Review and revise care plans as necessary to reflect changing needs and circumstances.\
  • Ensure compliance with regulatory requirements and industry standards.\

Location: Leeds, UK

\

Shift Patterns: Monday-Friday 9:00am-3:00pm, alternate weekends 7:00am-2:00pm or 4:00pm-10:00pm.
