About the Role

We are seeking a highly motivated and compassionate Support Worker to join our team at Northern Healthcare. This is an excellent opportunity for individuals who want to make a positive impact on the lives of our residents.

As a Support Worker, you will play a vital role in enhancing the lives of our residents, supporting their families, and being an integral part of a wider mental health support network within your local community.

You will be responsible for providing person-centred care and support to our residents, helping them to develop daily living skills and promoting independence. Your goal will be to empower them to live a full and meaningful life.

The ideal candidate will have excellent communication and interpersonal skills, with the ability to build strong relationships with our residents, their families, and other healthcare professionals.

Main Responsibilities

  1. Provide personal care and support to residents, including assistance with daily living tasks such as cooking, appointments, medication management, and money management.
  2. Encourage and support residents to re-learn skills and tackle new ones in a safe and supportive environment.
  3. Collaborate with the multi-disciplinary team to develop and implement individualised care plans that meet the unique needs of each resident.
  4. Maintain accurate records and report in a factual, person-centred approach.
  5. Escalate concerns or issues in a timely manner.
