**Salary:** £25,154 per annum, offering a competitive salary for the role of a Health and Wellbeing Practitioner.

**Job Description:

The successful candidate will deliver client-centered treatment, managing a caseload and providing harm reduction advice and guidance. This role involves assessing, planning, and delivering structured 1:1 and group-work sessions for substance misuse and low-level mental health service users.

Key responsibilities include developing and contributing to the Substance Misuse and general Health and Wellbeing care needs of service users, supporting them in the development of release plans appropriate to their needs.

The practitioner will ensure substance misuse and mental health groups, as well as Health and Wellbeing programs, are facilitated to the standard required by The Forward Trust and in accordance with relevant needs.

**Required Skills and Qualifications:
  • Demonstrable experience of carrying out risk assessments, comprehensive assessments, and designing SMART care plans.
  • Significant experience of psychosocial interventions, including motivational interviewing techniques in both 1:1 and therapeutic group settings.
  • A thorough knowledge of and commitment to the 12-step process of recovery from addiction.
  • Experience of providing counseling/CBT services to prisoners or those with Health and Wellbeing issues.
