Are you passionate about delivering client-centered treatment and making a positive impact in the lives of individuals struggling with substance misuse and mental health issues?

The Forward Trust, a leading social enterprise with charitable status, is seeking an experienced Health and Wellbeing Practitioner to join our team. As a key member of our multidisciplinary team, you will be responsible for managing a caseload, providing harm reduction advice and guidance, assessing, planning, and delivering structured 1:1 and group-work sessions for service users.

You will work closely with other Health and Wellbeing agencies within the prison and community settings to provide transitional support and prepare service users for release. Your role will also involve developing and contributing to the Substance Misuse and general Health and Wellbeing care needs of service users, supporting them in the development of release plans that cater to their individual needs.

To succeed in this role, you will require significant experience of carrying out risk assessments, comprehensive assessments, and designing and implementing SMART care plans. You should have a thorough knowledge of psychosocial interventions, including motivational interviewing techniques, and be committed to the 12-step process of recovery from addiction.
