Job Opportunity: Support Worker

We are seeking a dedicated and compassionate Support Worker to join our team in providing high-quality support to individuals with learning disabilities or autism in our supported living services.

As a Support Worker, you will work collaboratively with service users, their families, and wider support networks to promote their health, wellbeing, and independence. You will have the opportunity to act as a Key Worker, supporting individuals to achieve their goals and aspirations.

Key Responsibilities:

  • Providing direct care and support to individuals with learning disabilities or autism
  • Developing and implementing individual support plans to promote independence and community connections
  • Working collaboratively with multidisciplinary teams to ensure seamless support
  • Acting as a Key Worker to support individuals to achieve their goals and aspirations

Requirements:

  • Relevant experience of supporting individuals with learning disabilities or autism
  • Ability to work collaboratively with service users, families, and wider support networks
  • Strong communication and interpersonal skills
  • Ability to develop and implement individual support plans
