About the Role

We are seeking a compassionate and dedicated Community Care Assistant to join our team at Achieve Together. As a key member of our team, you will play a vital role in supporting individuals in their homes and local communities.

Job Summary

Key Responsibilities
  • Provide individualised support to service users in their homes and local communities
  • Assist with daily living tasks, personal care, and domestic duties
  • Encourage and enable service users to participate in activities that promote their physical and mental well-being
  • Maintain accurate records and reports as required
Requirements

To be successful in this role, you will need:

  • Excellent communication and interpersonal skills
  • A caring and empathetic approach to supporting vulnerable adults
  • The ability to work flexibly and as part of a team
  • Basic IT skills and the ability to learn new systems quickly
