About This Opportunity

We are seeking a highly skilled and dedicated Health and Wellbeing Connector to join our team. As a key member of our organization, you will play a vital role in promoting social prescribing and its role in self-management.

Key Responsibilities:
  • Develop strong links with all local agencies to encourage referrals and raise awareness of social prescribing.
  • Work in partnership with local agencies to reduce pressure on statutory services, improve health outcomes, and enable a holistic approach to care.
  • Facilitate introductions to community groups, activities, and statutory services, ensuring individuals are comfortable.

You will help people explore personal health budgets as a way of providing funded, personalised support for independence, including gaining skills for meaningful employment.

Requirements:
  • Become a trusted partner, providing non-judgmental support, empathy, and respecting diversity and lifestyle choices.
  • Foster a strength-based approach focusing on individuals' assets.
  • Help people maintain or regain independence through living skills, adaptations, enablement approaches, and simple safeguards.

A successful candidate will be able to work collectively with all local partners to ensure community groups are strong and sustainable.

Salary:

£35,000 - £45,000 per annum
