Job Summary
We are looking for a highly motivated and skilled Practice Nurse to provide exceptional patient care and support our team in delivering high-quality healthcare services. The successful candidate will have excellent clinical knowledge and skills, as well as strong communication and interpersonal skills.
Main Responsibilities
- Provide routine nursing care to patients, including wound care, ECGs, and venepuncture.
- Work closely with the practice team to deliver high-quality patient care and support.
- Develop and implement health promotion and well-being programs.
- Manage and support patients with chronic diseases, including diabetes, hypertension, and asthma.
- Prescribe medication and provide guidance to patients on medication use.
- Participate in local initiatives to enhance service delivery and patient care.
Requirements
- Registered Nurse with a strong clinical background.
- Experience in primary care and chronic disease management.
- Excellent communication and interpersonal skills.
- Ability to work autonomously and as part of a team.
- Flexibility to work outside of core office hours.
