About the Role
We are seeking caring and compassionate individuals to join our team as Care Professionals in busy teams within local care homes in and around Staffordshire. As a Care Professional, you will work in partnership with other health professionals and agencies to meet the requirements of the residents, promoting their comfort and independence.
Main Responsibilities
- Promote patient-focused care and assist residents with activities of daily living.
- Deliver support plans and safeguard vulnerable people.
- Contribute to outcome-focused reviews and encourage individuals to use equipment and assistive technology.
- Facilitate transitions to alternative services and support if needed.
- Engage in organisational learning and development, and provide excellent patient care and support.
- Record care and complete documentation and feedback.
