Youth Accommodation Coordinator

Homes Plus is seeking a Youth Accommodation Coordinator to join our team in Lancashire. As a key member of our staff, you'll play a vital role in ensuring the smooth operation of our Young Person's Supported Accommodation projects.
Key responsibilities include:
- Undertaking detailed needs assessments to identify the support requirements of each young person
- Providing expert guidance and advice to help them overcome challenges and achieve their goals
- Working closely with a multi-disciplinary team to deliver a cohesive service
- Safeguarding and risk management to protect the well-being of our customers
- Developing and implementing personalized support plans to help young people transition to independent living
- Fostering strong relationships with young customers, colleagues, and external agencies through effective communication and collaboration
This full-time position (36.25 hours/week) is part of a 7-day rota. A satisfactory DBS check and two professional references are required.
To succeed in this role, you'll need experience working with young people, excellent customer service skills, and local knowledge. IT proficiency is essential for efficient record-keeping and communication.
