About the Role

Priory Group is seeking a highly skilled and compassionate Social Worker to join our team at Priory Hospital Bristol. As a key member of our therapies team, you will play a vital role in delivering high-quality care to individuals with complex mental health needs.

Key Responsibilities

  • To provide person-centered care, ensuring the rights and dignity of service users are respected and upheld.
  • To work collaboratively with the Multi-Disciplinary Team to develop and implement effective care plans, focusing on prevention and early intervention.
  • To conduct thorough assessments, care planning, and tribunal reports to determine eligibility for social care services.
  • To identify and address social exclusions, their causes, and effects on mental wellbeing and mental health.
  • To empower service users to take control of their lives, making sustainable support arrangements and accessing social care and health resources.
  • To work in partnership with service users, carers, and other stakeholders to promote empowerment, equality, and social inclusion.
  • To support service users in planning, aftercare, and community support, ensuring a smooth transition to community-based services.
  • To promote carers' rights and access to assessments and resources.
