Job Title: Senior Continuing Care Assessor

Job Summary:

We are seeking a highly motivated and experienced Senior Continuing Care Assessor to join our team at NHS West Yorkshire Integrated Care Board. As a key member of our Continuing Care team, you will be responsible for assessing individuals against the National Framework for NHS Continuing Healthcare and Funded Nursing Care.

Main Responsibilities:

  • Undertake complex Continuing Healthcare assessments, decisions for eligibility, and review eligibility and appropriateness of commissioned care arrangements.
  • Lead and support others in identifying individuals' care needs, coordinating complex cases, and providing professional advice to patients, carers, and multi-agency colleagues.
  • Ensure individuals engage effectively with patients/carers to plan, assess, implement, and evaluate care programmes, providing specialist advice and leadership to junior staff.
  • Work with patients/clients, junior staff, and families to promote independence through Personal Health Budgets and the personalisation agenda.
  • Contribute to the protection of individuals from abuse, working within adult and child safeguarding policies.

Requirements:

  • Registered Clinical Assessor (part 1 of the Nursing and Midwifery Council (NMC) register as a registered Clinical Assessor.
  • Significant post-qualification experience of direct patient care involving assessment of people with complex health needs.
  • Knowledge of the National Framework for Continuing Healthcare and the Mental Capacity Act and associated guidance/policies relevant to the post.
  • Substantial communication skills, able to adapt complex information, and articulate care needs and recommendations of healthcare requirements.
