About the Role

We are seeking a Clinical Care Specialist to join our team at our Primary Care Hub in Sefton. As a key member of our team, you will work closely with patients and healthcare professionals to deliver high-quality patient care.

The successful candidate will have a degree in Pharmacy and experience of working in urgent care and/or primary care. You will also have a minimum of 2 years post-qualification experience and in-depth therapeutic and clinical knowledge.

We offer a competitive salary of £62,500 per annum, 27 days annual leave, and opportunities for Continue Professional Development. If you are passionate about delivering high-quality patient care, we encourage you to apply for this role.

Key Responsibilities
  • Work with patients and healthcare professionals to deliver high-quality patient care
  • Contribute to updating and implementing prescribing policy
  • Reconcile medicines following discharge from hospitals and intermediate care
