Care Coordinator

We are seeking a highly organized and communicative Care Coordinator to support our Chester Branch on a full-time basis, working 37.5 hours per week.
As a Care Coordinator, you will be responsible for coordinating the rota for care calls, maintaining accurate and detailed records, and communicating with colleagues and clients regarding their planned visits.
You will also support the on-call service on a rotational basis and build and maintain positive relationships with our clients, family members, colleagues, and other professionals.
Key responsibilities include introducing new employees to the branch, coordinating the rotas for our care staff, and effectively communicating rota changes to colleagues and clients.
You will be part of a truly caring and professional team that supports you as well as we would our clients.
Essential Skills
Excellent communication and organizational skills are essential for this role, as well as the ability to work effectively in a team environment.
You will also need to be able to drive and have access to a car.
