About the Role

We are seeking an enthusiastic and visionary nurse to join our Integrated Hospital Discharge team at Princess Alexandra Hospital. As a key member of our team, you will play a crucial role in enhancing patient experience of discharge planning services.

Main Responsibilities
  • Plan, provide, and maintain effective Discharge Planning care to patients/clients and their relatives.
  • Develop and enhance knowledge and skills that will directly influence consistent standards of Discharge Planning practice.
  • Work collaboratively with patients, carers, relatives, and system partners to ensure seamless discharge planning.
  • Support the development of student nurses and junior staff, creating an atmosphere conducive to learning and practice development.
