Discharge Planning Specialist

We are seeking an enthusiastic and visionary nurse to join our Integrated Hospital Discharge team at The Princess Alexandra Hospital NHS Trust. As a Discharge Support Nurse, you will play a key role in enhancing patient experience of discharge planning services, ensuring that patients' time and wishes are valued.
Main DutiesThe Integrated Hospital Discharge Team provides expert discharge planning advice and support for patients across the trust who require support from community services on discharge. Your main duties will include:
- Planning, providing, and maintaining effective Discharge Planning care to patients/clients and their relatives.
- Continuously developing and enhancing knowledge and skills that will directly influence consistent standards of Discharge Planning practice.
- Working with the ward-based and Integrated Hospitals Discharge team to support the development of student nurses and junior staff, creating an atmosphere conducive to learning and practice development.
The Princess Alexandra Hospital NHS Trust provides a full range of general acute, outpatient, and diagnostic services at The Princess Alexandra Hospital in Harlow, the Herts and Essex Hospital in Bishop's Stortford, and St Margaret's Hospital in Epping. We employ 3,500 staff and serve a local population of around 350,000 people living in west Essex and east Hertfordshire.
