Job Summary

We are seeking an experienced and skilled Discharge Support Nurse to join our Integrated Hospital Discharge team at The Princess Alexandra Hospital NHS Trust.

About the Role

The successful candidate will be responsible for planning, providing, and maintaining effective discharge planning care to patients and their relatives. This will involve working collaboratively with patients, carers, and relatives, as well as system partners, to ensure seamless transitions of care.

Main Duties
  • Develop and implement effective discharge planning strategies to ensure patient-centered care.
  • Collaborate with multidisciplinary teams to identify and address patient needs.
  • Provide education and support to patients and their families to promote self-management and independence.
  • Contribute to the development of policies and procedures to improve discharge planning practices.
  • Participate in quality improvement initiatives to enhance patient experience and outcomes.
