Job Overview

We are seeking an experienced Care Practitioner Specialist to join our team in Leeds. The successful candidate will work closely with the multi-disciplinary team to provide holistic support to adult service users.

About the Role

This is an exciting opportunity to work in a dynamic and rewarding environment, providing flexible outreach support to individuals with complex needs. As a Practitioner Outreach Worker, you will be employed by Waythrough (formerly Humankind) to support higher need service users who require a more personalized approach.

The ideal candidate will have a full clean UK driving licence and access to their own vehicle, as they will be required to conduct home visits and meet clients in the community.

Key Responsibilities
  • Assess and engage with individuals identified as requiring a more flexible approach to support, including those with multiple needs and comorbidities.
  • Manage a service user caseload and review ongoing care and treatment, liaising closely with specialist workers and family members as appropriate.
  • Conduct home visits, support clients to appointments with various services, and meet individuals in the community.
