Key Responsibilities:

  • Coordinate care plans for patients, taking into account their unique needs and preferences.
  • Collaborate with healthcare professionals to ensure seamless transitions and high-quality care.
  • Provide emotional support and guidance to patients and their families, promoting comfort and well-being.

Requirements:

  • Degree in Social Work or related field (or equivalent experience).
  • Excellent communication and interpersonal skills.
  • Able to work effectively in a fast-paced environment.

Salary Range:** £25,000 - £35,000 per annum, depending on experience.
