Job Description

We are seeking a highly skilled and experienced Specialist Care Coordinator to join our team at Queen Victoria Hospital NHS Foundation Trust. This is an exciting opportunity for a dedicated individual to work as part of a small team, driving the implementation of national best practice guidance for lower limb care.

The successful candidate will be responsible for assessing, diagnosing, implementing, planning, and reviewing care needs for patients with lower limb wounds. They will work closely with multidisciplinary teams, including nursing homes, district nurses, practice nurses, and acute nurses, to provide specialist advice and support.

Key responsibilities include:

  • Assessing and diagnosing lower limb wounds
  • Developing and implementing care plans
  • Providing specialist advice and support to multidisciplinary teams
  • Undertaking wound therapies, such as negative pressure wound therapy and larval therapy

A full UK driving licence is essential for this role, as you will be required to visit patients in various environments.

Estimated salary: £38,000 - £45,000 per annum, depending on experience.
