About the Role

We are seeking an experienced and skilled Advanced Nurse Practitioner to join our team at Haden Vale Medical Practice. As a key member of our multidisciplinary team, you will be responsible for delivering high-quality patient care and working closely with our GPs and other healthcare professionals to provide comprehensive care to our patients.

Key Responsibilities
  • Manage patients presenting with a range of acute and chronic medical conditions, providing expert advice and care.
  • Provide advanced nursing care to patients, including health promotion, chronic disease management, and health prevention.
  • Work closely with our GPs and other healthcare professionals to provide comprehensive care to our patients.
  • Develop and implement care plans to meet the individual needs of our patients.
  • Conduct health assessments, take medical histories, and perform physical examinations as required.
  • Order and interpret diagnostic tests, including blood tests and other investigations.
  • Prescribe medications and treatments as necessary, in accordance with our practice policies and procedures.
  • Provide education and support to patients and their families on health-related topics.
  • Participate in quality improvement initiatives and contribute to the development of our practice policies and procedures.
Requirements
  • Registered Nurse with a postgraduate diploma or degree in Advanced Practice.
  • Qualified Nurse Prescriber with experience of prescribing and medication management.
  • Experience of working in a primary care environment, preferably in a similar role.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, GPs, and other healthcare professionals.
  • Ability to work autonomously and as part of a team, with a flexible and adaptable approach to work.
  • Commitment to ongoing professional development and a desire to stay up-to-date with the latest clinical guidelines and best practices.
