**Job Summary**

We are seeking a skilled Occupational Therapist/Physiotherapist to join our team in a community role.

**Key Responsibilities:**

  • Contribute to the delivery of high standards of healthcare to patients in their own homes, Intermediate Care Beds, or a rehabilitation Unit.
  • Assess and prescribe treatment packages for patients with long-term conditions, the frail & elderly, palliative care, and rehabilitation needs.
  • Formulate and execute individualised occupational/physiotherapy treatment plans in collaboration with the integrated team and other agencies.

**Requirements:**

  • Proactive, holistic case management and care skills.
  • Ability to assess and prescribe treatment packages.
  • Experience in formulating and executing individualised treatment plans.

**About the Role:**

This is a full-time position with a minimum of 3 months, with the possibility of extension.
