About the Role

This is an exciting opportunity to work in a dynamic and supportive environment, where you will have the chance to make a real difference in the lives of our patients. As a Community Mental Health Practitioner, you will be responsible for assessing and delivering specific therapeutic interventions to support clients with complex mental health needs. You will work closely with other professionals to provide a comprehensive and coordinated approach to care.

You will have the opportunity to work collaboratively with members of the multi-disciplinary team, including primary and secondary care services, social and voluntary care systems, and patients and carers. This will enable individuals to achieve their optimum physical, psychological, and social wellbeing and improve the lives of people who access healthcare services in the locality.

Main Responsibilities
  1. Conduct assessments and deliver specific therapeutic interventions, including liaison with other professionals regarding progress and outcomes.
  2. Have responsibility for documenting the plans of care and ensuring that plans remain current and that standards for record-keeping are adhered to.
  3. Delegate aspects of the patient treatment plan to other members of the team, whilst maintaining overall responsibility for patients care, where appropriate.
  4. Attend regular MDT meetings with the team manager to address caseload management issues.
  5. Support clients with their physical health awareness and some health education.
  6. Allocate diary time to cover the Duty rota.
  7. Provide managerial supervision to colleagues in the team.
Requirements

To be successful in this role, you will need to have relevant experience of working with adults with mental health needs, or clear evidence of substantial experience of working with individuals in crisis and skills that are transferable to both community/in-patient settings. You will also need to have evidence of continuing professional development and sound knowledge of national agenda for mental health.

You will need to be registered with a professional body, such as the NMC, UKCP, HCPC, or BACP, and have management training. You will also need to be able to travel independently across the county.
