Adult Mental Health Practitioner

We are seeking a highly skilled and experienced Adult Mental Health Practitioner to join our team at Lincolnshire Partnership NHS Foundation Trust. As a key member of our Community Mental Health Team, you will play a vital role in delivering high-quality care to individuals with complex mental health needs.
Your primary responsibility will be to act as the lead in the care and treatment of clients with complex mental health needs, demonstrating evidence-based and innovative practice. You will be responsible for documenting care plans, ensuring they remain current, and adhering to standards for record-keeping. Risk assessment and risk management are essential requirements for this role.
You will work collaboratively with the multi-disciplinary team to ensure effective and appropriate clinical decision-making, both within the Community Mental Health Team and with external agencies. You will also be responsible for maintaining accurate and timely clinical records and coordinating and monitoring those of the team.

Key Responsibilities- Act as the lead in the care and treatment of clients with complex mental health needs
- Document care plans and ensure they remain current
- Maintain accurate and timely clinical records
- Coordinate and monitor clinical records of the team
- Work collaboratively with the multi-disciplinary team to ensure effective and appropriate clinical decision-making
- Develop effective and supportive links with other health and social care staff
- Relevant experience of working with adults with mental health needs
- Experience of working with people with severe and enduring mental illnesses
- Evidence of continuing professional development
- Sound knowledge of national agenda for mental health
- Sound knowledge of clinical/risk assessment and understanding of Information Governance principles
