Job Title: Adult Advanced Practitioner

We are seeking an experienced and skilled Adult Advanced Practitioner to join our team at Lincolnshire Community Health Service NHS Trust.

The successful candidate will be responsible for assessing, treating, and referring patients with minor illnesses, as well as managing long-term conditions and prescribing medication appropriately.

Key responsibilities include:

  1. Conducting telephone triage assessments and providing guidance to patients.
  2. Assessing and managing patients with minor illnesses and long-term conditions.
  3. Prescribing medication and reviewing its use to ensure safe and effective treatment.
  4. Requesting and interpreting investigations to inform patient care.
  5. Referring patients to other members of the multidisciplinary team, secondary care, and other statutory and partnership organizations as needed.

The ideal candidate will have a strong background in urgent care, non-medical prescribing, and leadership and management. They will also have excellent communication and interpersonal skills, with the ability to work autonomously and as part of a team.

This is a bank position, working with our friendly team of GPs and healthcare support workers. The successful candidate will have the opportunity to support home visiting and urgent treatment centre work as required.

Person Specification
  • MSC in Advanced Clinical Practice.
  • Non-Medical Prescribing qualification.
  • Significant urgent care experience.
  • Level 6 or 7 qualification in History Taking and Clinical Assessment.
  • Safeguarding Level 3 or willingness to undertake.
  • Able to demonstrate multi-professional working and working across organizational boundaries.
  • Management of chronic diseases.
  • Experience in telephone triage.
