Key Responsibilities:- Support Senior Colleagues: Assist senior team members with prioritization, allocation, planning, and review of work in accordance with policies, procedures, and statutory requirements, ensuring effective budget management and maximizing service efficiencies.
- Contribute to Service Monitoring and Evaluation: Participate in service monitoring and evaluation, undertaking needs analysis, reviewing availability of resources, and developing service practice, policies, and strategies to maintain high-quality services.
- Provide Supervision and Direction: Offer supervision, direction, and coordination of workloads, support, and guidance, appraisal, and professional development, as well as support social work students on first and final placements.
- Undertake Service User Assessments: Conduct clear and detailed service user, carer assessments, and interventions in accordance with relevant statutory requirements, minimizing risks, and considering service user needs and available options.
- Manage Complex Cases: Manage high-complex cases, including planning and review, promoting independence and early intervention, as well as the personalization agenda and self-directed support to minimize risks and improve quality of life for service users and their carers, ensuring targets are met within agreed timescales.
- Work with Individuals and Families: Collaborate with individuals, families, carers, and communities to help them make informed decisions, enabling them to clarify their needs, construct value for money care packages, and commission a range of appropriate services, as well as respond to their feedback regarding service provision.
- Ensure Statutory Responsibilities: Assist in ensuring that statutory responsibilities are met and fulfilled by the team, ensuring that quality practice standards are achieved in delivering high-quality services, promoting the use of evidence and theory to support practice in complex and changing circumstances.
- Undertake Preventative Work: Conduct preventative work and risk assessments utilizing community resources and services such as reablement and telehealthcare.
- Manage Complex Transitions: Manage complex transitions cases and chair multi-disciplinary team meetings as appropriate to specialized field/local structures.
- Maintain Knowledge and Skills: Maintain up-to-date knowledge of best practice, policy, and procedure in relation to vulnerable people, as well as keeping abreast of wider service developments to enhance the service accordingly to satisfy internal and external demands.
- Travel Requirements: Ability to travel independently to all required areas by the job's responsibilities.
