Regional Care Lead

We are seeking an experienced and skilled Regional Care Lead to join our team at Home Care Direct. As a key member of our staff, you will play a crucial role in coordinating care services for our clients. This is a full-time position based in Chesterfield, East Midlands, with opportunities for hybrid working.
Key Responsibilities- Coordinate care services for clients, ensuring seamless delivery and exceptional outcomes.
- Develop and maintain relationships with healthcare professionals, clients, and their families.
- Liaise with internal teams to ensure effective communication and collaboration.
To be successful in this role, you will need:
- A Level 3 qualification in health and social care or a related field.
- Proven experience working in the health or social care industry.
- Excellent communication and interpersonal skills.
- High level of computer literacy, including Microsoft Office.
- Ability to work under pressure and manage multiple tasks simultaneously.
