About the Role

We are seeking a highly motivated and experienced Practice Nurse to join our team at Wollaton Park Medical Centre. As a key member of our healthcare team, you will be responsible for providing high-quality nursing care to our patients and contributing to the delivery of excellent patient outcomes.

Key Responsibilities
  • Provide and maintain a high standard of nursing care to patients, in conjunction with nursing team colleagues.
  • Focus on delivering evidence-based practice for patients with long-term conditions and providing preventative health care to the practice population.
  • Provide nursing care to practice patients, including primary care treatment, preventative care, screening, and education.
  • Take an active role in supporting the practice to maximize achievement against local and national targets in respect of the PMS contract, QOF, and Enhanced Services.
  • Communicate effectively with integrated nursing teams, patients, and carers, recognizing their needs for alternative methods of communication.
  • Take responsibility for own developmental learning and performance, including participating in supervision and maintaining a record of personal development.
  • Work with management on new training requirements and recognize the roles and responsibilities of individuals working in the primary healthcare team.
  • Use personal security systems within the workplace according to practice guidelines and identify risks involved in work activities, undertaking them in a way that manages the risks.
  • Apply infection control procedures and maintain work areas in each clinical room, ensuring they are clean, safe, and free from hazards.
  • Understand, apply, and manage the principles of the cold chain, ensuring safe storage, rotation, and disposal of vaccines and drugs within the area of responsibility.
  • Know the general principles of first aid and resuscitation and be aware of statutory child health procedures and statutory local guidance and referral criteria.
  • Know the health and safety policies and procedures within the workplace, including fire procedures, maintaining documentation, monitoring, and maintaining equipment and furniture within the area of responsibility.
  • Use the computer safely and identify risks to health of microbiological and chemical hazards within the working environment according to the Control of Substances Hazardous to Health.
  • Work within the guidelines set by CQC to ensure that the Practice is safe, efficient, and provides the best possible care for patients.
  • Act in ways that recognize the importance of people's rights, interpreting them in a way that is consistent with procedures, and respect the privacy, dignity, needs, and beliefs of patients and carers.

Qualifications

  • Essential:
  • Registered Nurse with sufficient demonstrable experience post qualification.
  • GCSE grade A to C in English and Maths.
