Job Title: Advanced Practitioner - Primary Care

We are seeking an experienced Advanced Practitioner to join our team in Primary Care. As an Advanced Practitioner, you will be at the forefront of healthcare innovation, utilising your advanced clinical skills to provide the highest levels of care to all patients.

Main Responsibilities:
  • Undertake structured surgeries working in an autonomous role, including the management of patients presenting with new acute problems or exacerbation of chronic disorders.
  • Assess, diagnose, treat, refer or signpost patients who attend surgery with undifferentiated or undiagnosed conditions.
  • Be a training and mentoring resource for peers and hosted trainees.
  • Manage daily surgeries comprising face-to-face consultations, home visits and telephone consultations.
  • Manage results and workflow in line with practice policy and procedures.
  • Work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures.
Requirements:
  • NMC Registered Nurse
  • Completed MSc in Advanced Clinical Practice
  • Level 7 V300 Non-Medical Prescribing or equivalent
  • Completed a minor ailment and injury module at BSc/MSc level
  • Evidence of PREP
Experience:
  • Several years post registration
  • Experience of working in general practice
  • Experience in managing long-term conditions
  • Experience in management of diseases and undiagnosed health conditions
  • Experience in minor illness, clinical decisions, telephone triage
  • Experience of working autonomously
