Primary Care Network Liaison Officer

We are seeking a dedicated and compassionate Care Coordinator to join our team at Haxby Group's Primary Care Network.
About the Role:This is an exciting opportunity to work collaboratively with our team to deliver high-quality care to our patients. As a Care Coordinator, you will be responsible for developing and implementing care plans, ensuring that our patients receive the best possible support and treatment.
- Key Responsibilities:
- Develop and review care plans to include being involved in the introduction and changes to care plans.
- Ensure care plans and advanced care plans are up to date and shared with relevant people.
- Inform the Lead Clinician of any delays in assessment or delivery of care needs.
Requirements:
Qualifications and Training:- Achieved grade C or above in English and Maths GCSE or equivalent.
- NVQ Level 3 Health and Social Care (or relevant experience).
- Formal training working with vulnerable individuals.
- Flexible.
- Experience of working without direct supervision.
Salary: £25,000 - £30,000 per annum.
