Social Worker

Liquid Personnel is seeking an experienced Social Worker to join their team as a Care Manager with a rehabilitation focus.
About the Role:
- Conduct comprehensive assessments of adults in need of social care intervention, taking into account their psychological, cultural, and emotional needs.
- Collaborate with healthcare professionals to develop effective discharge plans, ensuring that service users are safely discharged and have the necessary support in place.
- Provide support and guidance to service users to help them regain their independence through rehabilitation.
- Manage a caseload of service users, regularly reviewing and updating care plans as necessary.
- Work as part of a multi-disciplinary team, maintaining effective communication and cooperation with colleagues from various disciplines.
What We Are Looking For:
- Social Work England registration
- Eligible to work in the UK
- Hold a full UK licence
- Post-qualified experience
- Degree level or equivalent in Social Work
Salary and Benefits:
